  • Prompt: Describe in detail the feelings and thoughts of a patient with schizophrenia. It is impossible to describe the feelings and thoughts of a person with schizophrenia in "one word," because this illness is a disorder of a shattered mirror. The inner world of such a patient is not simply "fear" or "sadness"—it is a **fundamental loss of self-evidence**.To understand it, imagine waking up in a movie where all the actors know the script, but you don't. Yet at the same time, you are absolutely certain that the movie is being made *about* you. Here is a detailed breakdown of this state, layer by layer. 1. Thinking: "Porridge of Broken Glass"A healthy person thinks linearly: "Saw rain -> Thought about an umbrella -> Remembered the umbrella is broken." In a patient with schizophrenia (especially during an acute phase), this chain breaks. Fragmentation (Derailment):** Thoughts flow in parallel. The patient may be thinking about space, while simultaneously feeling that their hand is made of cotton, and hearing the neighbor transmit secret signals to them through coughing. All of this mixes into one "hodgepodge." - **Tangentiality:** There is logic, but it is understandable only to the patient themselves. For example, they won't drink tea because "tea is liquid time, and time flows away, which means I will flow away into the cup."Thought Withdrawal (Sperrung):** Suddenly, there is complete silence in the head. This frightens them no less than the noise. The person feels that their brain has been forcibly "turned off." 2. Feelings: Emotional Ice and Flame This is clinically called *apathy-aboulia syndrome** (flattening of emotional affect), but that sounds dry. In reality, it is the feeling that there is a thick, frosted glass between you and the world.- **Woodenness:** You know you *should* love your mother or rejoice in the sun. You *remember* the feeling of joy, but you cannot *experience* it. Emotions become flat. This state is often described as "I am dead inside, even though I walk and talk."- **Paradoxical Reactions:** At the same time, there is an explosion of emotion over *trivial* things. A person may look blankly at the death of a loved one, but burst into tears because a glass is placed incorrectly on the table. This happens because real events seem unreal, while unreal ones (the voices) feel absolutely alive. 3. Perception of the World: Sensory Assault In schizophrenia, the brain stops filtering out noise. Normally, our brain muffles the sound of the refrigerator and the noise of the street. In a patient, **all sounds are equally loud and important**. - The light from a lamp can seem as blinding as the sun. - A whisper behind the wall is heard as if the person is speaking directly into their ear. - Colors become unnaturally bright or, conversely, dull to the point of blackness. The world is perceived as a hostile, too-dense substance that presses down on the skin.4. The Most Frightening Part: Delusions and "Voices" (Inner Reality) is not about hallucinations of "little green men." This is an **intimate invasion**. **Commentary Voices:** The most common type. Two or three voices (or more) discuss the patient's every action as if in a live broadcast. *"Look, he picked up the spoon... He thinks we don't see him... He is dirty..."*
